Position Duties Include:

• Manage and schedule your visits. You'll travel around the area to provide 1:1 nursing services to patients in their home
• Collaborate with an interdisciplinary team to help provide the best care possible to our patients

Qualifications

Qualified applicants need a valid Minnesota LPN license and interest in visiting patients in their homes in the Shoreview and surrounding area. This position is well suited to an individual that enjoys direct patient contact, supporting a dedicated care team doing good in our communities.
